Applicant, if you have owned a dog or cat in the last 5 years, please have your attending vet complete and submit this form.
PLEASE NOTE: We only need this basic information and not the pet’s whole health history.

Please send the FORM ONLY, not the medical history of each pet. Only send records to the above email if additional clarity is required in special circumstances.

If you are unable to complete the form online, download the PDF version of the form and send the completed form back to us via email at vetrecords@lolin.org.
